  • For Your Appointment, please complet the following steps:

  • Call Retina Consultants of Nevada and Schedule your examination by phoning (702)369-0200.  You may look up the location nearest to you and a map to get to our office.

  • Bring your referral form with you to the office.
  • Make a list of all of your medications and bring it with you
  • Bring the name of your primary physician, address and phone number
  • Bring the name of your ophthalmologist/optometrist with you
  • Bring your insurance card(s) with you
  • For your first appointment only:

  • Print  the patient information form, fill it out and bring it with you (go to patient information form)

  • Print the privacy statement, read it, fill out the form, sign it and bring it in with you

 

Our practice is a specialty eye practice limited to treatment of diseases and surgery of the retina and vitreous. Patients that are new to our practice will have a comprehensive consultation as well as possible additional testing ordered by the doctor.

The length of your appointment can vary based on the severity and complexity of your eye condition. As we are a referral practice, with frequent and unpredictable emergencies sent to us for our immediate attention, our appointment schedule at times may be delayed . The average appointment length is about two hours. Both of your eyes will be dilated and can affect your ability to drive. It is recommended that you arrange for transportation to and from our office.

Although we make every effort to provide translators, we are not able to provide one on every visit.  It would be best if you are able to bring a translator with you on your examination.

 

We accept most health insurance plans, including Medicare and Medicaid.  Be sure to bring your insurance cards and referral forms for your visit.  Also expect to pay your co-payment and any deductible amounts at the time of your appointment.  If special payment arrangements are necessary, please contact our office staff prior to your appointment.
